Chronic Condition Special Needs Plans (C-SNPs) | 2027

If you have diabetes, chronic heart failure or a cardiovascular disorder, ask about our C-SNPs.

These plans offer a monthly credit to buy OTC health & wellness items, plus healthy foods for qualifying members*. C-SNPs may also include lower copays than some of our other plans for specialist visits and certain prescription drugs.

There's a special enrollment period for enrolling in a C-SNP.

If you have a qualifying condition, you can use a one-time special enrollment period to enroll in one of these plans at any point during the calendar year.
