This directory provides information about Peoples Health current network providers.

This directory is for Peoples Health plan members. Peoples Health offers plans in all Louisiana parishes.

To request a hard copy of Peoples Health’s provider directory, please call our member service department at toll-free at 1-877-369-1907, seven days a week, from 8 a.m. to 8 p.m. TTY users should call 711. Peoples Health will mail a hard copy of the provider directory to you within three (3) business days of your request.

If you identify an error with any of the provider information in our online directory, use the Report Incorrect Information button on the provider’s search results screen or call member services.


This directory provides information about Peoples Health network providers.

The network providers in this directory have agreed to provide you with your health care services. You will have to choose one of our network providers in this directory to be your primary care physician (PCP). Generally, you must get your health care services from your PCP. You can search for PCPs in the Physician Search. Check the Search Only for Primary Care Physicians box, then enter your other search criteria.

You may go to any of our network providers in this directory; however, some services may require prior authorization. This means your provider needs to get approval from us before giving you the services. Our clinical staff will review your provider’s request and make a decision. Our staff uses nationally recognized criteria to make sure the service is appropriate for both your condition and the setting (inpatient, outpatient, etc.). The services that may need prior authorization are noted in the Evidence of Coverage.

Please note that some providers may not be accepting new patients. The directory provides this information.

This directory is updated weekly. Providers may have been added or removed from our network since this directory was last updated, or some providers may have changed whether they are accepting new patients. This means that some providers may no longer be in our network or there may be newer providers in our network that are not yet listed.

Peoples Health Choices 65 (HMO-POS), Peoples Health Choices Gold (HMO-POS), Peoples Health Secure Health (HMO-POS D-SNP) and Peoples Health Secure Complete (HMO-POS D-SNP) members: You must use network providers except in emergency or urgent care situations or for out-of-area renal dialysis. You may use your point of service (POS) benefit to see non-network providers for routine dental services only. If you obtain routine care, except covered dental services, from out-of-network providers, neither Medicare nor Peoples Health will be responsible for the costs.

Peoples Health Choices (PPO) and Peoples Health Patriot (PPO) members: You can use network providers or out-of-network providers. With the exception of emergency care and urgently needed care, it may cost more to get care from out-of-network providers. Most out-of-network services must be medically necessary. Out-of-network providers are under no obligation to treat Peoples Health Choices and Peoples Health Patriot members, except in emergencies. For a decision about whether we will cover an out-of-network service, we encourage you or your provider to ask us for a pre-service organization determination before you receive the service. You may also refer to your Evidence of Coverage for more information, including the cost-sharing that applies to out-of-network services.

Peoples Health Group Medicare (HMO-POS) members: You can use network providers or out-of-network providers. With the exception of emergency care and urgently needed care, it may cost more to get care from out-of-network providers. Some services may not be covered from an out-of-network provider. Most out-of-network services must be medically necessary and may need prior authorization. Out-of-network providers are under no obligation to treat plan members, except in emergency situations. How do you find Peoples Health providers that serve your area?

You can search for network providers in this directory using our provider search tools – Physician Search, Hospital Search, Other Provider Search, Routine Vision Providers Search and Fitness Center Search.

  • Use the Physician Search to find network doctors, including PCPs and specialists.
  • Use the Hospital Search to find network hospitals.
  • Use the Other Provider Search to find other network providers, for example, skilled nursing facilities and outpatient mental health providers.
  • Use the Routine Vision Providers Search to find a network optometrist for routine vision care (annual eye exam and eye glasses for vision correction).
  • Use the Fitness Centers Search to find a network fitness center in your area.

Each search tool helps you look for providers based on available search criteria.

What is a PCP and what does a PCP do for you?

Your PCP is your main contact for health care. Your relationship with your PCP is an important one because your PCP is responsible for the coordination of your health care and is also responsible for your routine health care needs. It is important for you to develop and maintain a relationship with your PCP. Your PCP can offer most of your care. Examples of PCPs are internal medicine, general practice and family practice doctors.

If you need specialist care or hospital services, your PCP can help set up services for you. You can also set up services yourself by getting care from any provider in our network. You may want to follow-up with your PCP after any specialist visits.

How do you choose a PCP?

If you did not choose a PCP during enrollment, we assigned one to you.

How can you switch to another PCP?

You may change your PCP for any reason, at any time. Also, it’s possible that your PCP might leave our network of providers and you would have to find a new PCP.

You may choose a new PCP, who is accepting new patients, from any of the providers listed in the Physician Search. Check the Search Only for Primary Care Physicians box, then enter your other search criteria.

You can change your PCP by logging in to MyPeoplesHealth at www.mypeopleshealth.com or by calling member services. If you need to choose a PCP or need more information, we can help.

PCP changes start the first day of the month after you make your request.

What should you do if you have bills from out-of-network providers?

If an out-of-network provider sends you a bill asking you to pay for covered services, you should not pay the bill directly. Ask the provider to bill us first. We may pay the provider for our share of the bill, and we will let you know if you must pay anything. If you have already paid the provider, we may reimburse you for our share of the cost. Please call member services with any questions.

What if you have a medical emergency?

Get help as quickly as possible. Call 911 for help or go to the nearest emergency room or hospital. Emergency care can always be obtained in or out of the service area from the nearest available provider. Emergency services are also covered whether a provider is in our network or not. Call for an ambulance if you need it. You do not need to get approval or a referral first from your PCP.

What if you need urgent medical services?

An urgently needed service is a nonemergency situation requiring immediate medical care but, given your circumstances, it is not possible or not reasonable to obtain these services from a network provider. The plan must cover urgently needed services provided out-of-network.

Are virtual visits (telehealth) covered?

Virtual visits are covered under your benefit plan. Not all medical conditions can be treated through virtual visits. The virtual provider will identify if you need to see an in-person doctor for treatment. How do I access my behavioral health benefits?

  • To access your behavioral health benefits, call the toll-free behavioral health number on your Peoples Health member ID card, Optum, 1-877-566-7913, seven days a week, from 8 a.m. to 8 p.m. TTY users may call 711. Or, visit www.liveandworkwell.com. When you call, a representative will check your eligibility and get basic information about you and your situation. Depending on the help you need, a clinician may talk with you and recommend a provider and treatment plan. If you prefer, your PCP may call and arrange a referral for you. Your personal information will be kept strictly confidential.
