You can use any of these vision providers to receive routine vision care. Plan members are entitled to a routine vision exam from one of these providers each year. Members are also entitled to one pair of glasses or contact lenses for vision correction purposes from these providers each year. Check your plan’s Evidence of Coverage for the costs associated with these services.

If you are looking for a vision provider who can provide you with medical vision care (which includes treatment for diseases of the eye and surgery of the visual pathways, including the eye, brain and areas surrounding the eye, including the eyelids; and treatment for glaucoma and cataracts), use the Physician Search tool to find an Ophthalmologist under your specific plan.
