Peoples Health Choices Select (HMO) has a $116* monthly plan premium.

This is in addition to any Medicare Part B premium you may have.

Medicare provides Extra Help to pay prescription drug costs for people who have limited income and resources. Extra Help is also called the “low-income subsidy” or LIS. People whose yearly income and resources are below certain limits can qualify for this help.

If you qualify, Extra Help will subsidize (help pay for) any portion of your plan premium that is for prescription drug coverage. It will also help pay for some of your prescription drug costs and your prescription drug yearly deductible (if applicable). The amount Extra Help pays for your prescription drugs counts toward your total out-of-pocket costs.

Some people automatically qualify for Extra Help and don’t need to apply. Medicare mails a letter to people who automatically qualify for Extra Help.

If you are a plan member and think you may qualify for Extra Help, you may call the Peoples Health Member Services department toll-free at (800) 222-8600, seven days a week, from 8 a.m. to 8 p.m. TTY users may call 711. If you contact Member Services on a weekend or a holiday, you may need to leave a message, but we will return your call within one business day.

If you qualify for Extra Help, the table below tells how much you will pay for the Peoples Health Choices Select (HMO) monthly premium. The Peoples Health Choices Select (HMO) monthly premium is for your medical coverage. There is no premium for your prescription drug coverage. Therefore, you will continue to pay $116 for your monthly premium regardless of your Extra Help level.
