Peoples Health Choices 65 (HMO) parish mapPeoples Health Choices Plus (HMO) is available to Medicare-eligible residents in the following Louisiana parishes: Ascension, East Baton Rouge, Livingston, St. Bernard, St. Charles, St. James, St. John the Baptist, West Baton Rouge.

With Peoples Health Choices Plus (HMO), you get more coverage than with Medicare alone for no monthly premium above what you pay now for Medicare Part B. In addition to medical healthcare services, Peoples Health Choices Plus (HMO) also offers Medicare Part D prescription drug coverage, preventive dental services and routine vision services for no additional monthly premium.
