Provider Network

Members of Peoples Health Choices 65 (HMO), Peoples Health Choices Plus (HMO), Peoples Health Choices Select (HMO), Peoples Health Secure Choice (HMO SNP), Peoples Health Secure Health (HMO SNP), Peoples Health Group Medicare (HMO-POS) and PHN Employee Plan have access to participating providers in the Peoples Health provider network. This network consists of 36 area hospital facilities and over 2,200 independent physicians. Over 400 of these physicians are primary care doctors.

Contracting with Peoples Health

Providers who would like to contract with a Peoples Health physician team, or existing providers who would like to switch physician teams or who have other non-standard contract requests, should submit a Letter of Interest or Letter of Request to the following address:

Please include the following in the Letter of Interest or Letter of Request:

• Provider’s specialty
• Area(s) serviced
• Physical address and phone number of the provider office/agency
• A list of services offered
• A copy of the provider's Curriculum Vitae
• Any area hospital(s) the provider is associated with (list hospital and status, for example, active, courtesy, temp, etc.)
• Indicate if the provider is board certified

All Letters of Interest or Letters of Request will receive an acknowledgement letter confirming receipt and informing the provider of when their contract request will be submitted for Peoples Health’s internal review process.
