We're dedicated to maintaining and improving your health and well-being. In an effort to do this, we recommend the following services:

Men and Women

  • Regular Primary Care Physician Visits
  • Annual Influenza (Flu) Immunization (age 50 and older) 
  • Pneumococcal (Pneumonia) Immunization (once at age 65 and every 5-6 years thereafter)
    • Cholesterol Screening - routine screening of men beginning at age 35 and women at age 45 in the absence of known risk factors. Screening at age 20-35 for men and 20-45 for women if the following risk factors are present. Check with your physician regarding the frequency of screening if you have risk factors.  
           • Diabetes
           • Family history of Cardiovascular Disease (CVD) before age 50 in male relatives and 60 in female relatives
           • Family history suggestive of familial Hyperlipidemia (elevation of lipids (fats) in the bloodstream)
           • Multiple Coronary Heart Disease (CHD) risk factors (tobacco use, hypertension)
    • Blood pressure measurement (every two years beginning at age 18 if systolic is less than 130 and diastolic is less than 85 and more frequent intervals for screening those with blood pressure at higher levels. Check with your physician regarding the frequency of screening.)
    • Colorectal Cancer Screening
      Men and women who are average risk should begin screening at age 50. If you are at greater risk, you may need to begin regular colorectal cancer screening at an earlier age. Check with your physician if you have risk factors.     
           • Fecal Occult Blood Test (FOBT) yearly
           • Sigmoidoscopy (every 5 years beginning at age 50) or
           • Colonoscopy (every 10 years beginning at age 50)

Women Only

  • Mammograms
         • Age 35-39: Baseline (one time)
         • Age 40-51: At least every two years
         • Age 52-69: Annually
         • Age 70-Over: Per physician/patient agreement
  • Pap Smear
         • Annually age 21-65
         • Age 66 and older: Per physician/patient agreement

Men Only

  • PSA (Prostate Specific Antigen) Test (beginning at age 50)
  • Digital Rectal Exams (annually beginning at age 40)

    Diabetic Patients

    • Glycosylated Hemoglobin – HbA1c (every three months)
    • Lipid Profile (annually)
    • Microalbuminuria (annually)
    • Retinal Eye Exam (annually)
    • Foot exam (annually)
    • Blood Sugar Checks – check with your physician regarding the frequency of these checks
