PHN Association has developed a Disaster Planning Toolkit to help you prepare disaster plans for your office, including plans for communicating with staff and patients during a disaster and resuming full operations following a disaster. We believe that these preparations can reduce recovery time, so we recommend that you start planning today.
The toolkit includes items which you can personalize to suit the individual needs of your office and staff. Electronic versions of each of these tools are available for download by clicking on the links below:
- Disaster Planning Checklist – a checklist of items to consider as you plan to preserve assets and communicate with your staff and patients.
- Tracking Map and Important Phone Numbers – the map can signal steps that need to be taken in advance of a storm hitting the area.
- Disaster Recovery Checklist – a guide of steps to follow upon your return from an evacuation.
- Full Circle Call Tree and Instructions - a diagram of your staff, their important phone numbers and the phone numbers of their alternate contacts. This diagram acts as a directional plan of whom will contact who in the event of a disaster.
- Instructions for setting up instant message groups at Yahoo.com to enable your staff to communicate when cell phones may not work.
- Instructions for creating a business card with critical telephone information for your staff or patients.
- In an effort to maintain communication with our providers in the event of a disaster occurrence, Peoples Health is gathering vital provider information. Please complete the Provider Disaster Communication Information Form to ensure that we have your most current contact information. You may return your completed form to your Provider Relations representative, or you may fax it to (504) 849-6903.
